Community Hope Concept’s health program provides low-income older adults with financial assistance for routine medical co-pays, deductibles, prescription drugs and supplies.  Eligible recipients are awarded benefits on a quarterly basis to pay for healthcare related expenses. Program service(s) are solely based on the availability of funding sources, and are considered a benefit, not as a right or entitlement.

Who is eligible for the Program Assistance Program?

​

To be eligible for this benefit program, you must be a resident of Houston, Texas with a valid Driver’s license or ID card issued by the State and meet ALL of the following requirements:

​

• Be age 60 or older and have a medically diagnosed chronic condition;

 

• Provide proof of income based financial need guidelines;

  • You have a current bank balance (savings and checking combined) under $1,920 per month, one person household (maximum earning no more than $23,000 a year), or

  • You have a current bank balance (savings and checking combined) under $2,580 per month, two person household (maximum earning no more than $31,000 a year):

• Provide proof of Medicare, Medicaid or SSDI benefits coverage.

If you meet the above eligibility guidelines and can provide proof of identity, residence, income and current health status, email our Intake Department at info@chopeconcepts.org to request an application.

Open Enrollment begins September 2-19, 2024.  Any applications received after the last day to apply will be placed on the waitlist for the next quarterly enrollment period.
